Prescription/Order Requirement:
A valid prescription or order from a doctor is required to bill your insurance company. Prescriptions and orders are required to include Diagnoses, Number of visits, and the end of duration date.

Insurance Disclaimer: Peach Studios LLC partners with Terri's Desk LLC to provide a quote of your benefits as a courtesy service only but it does not guarantee payment by insurance companies. Payment of benefits are subject to all terms, conditions, limitations, and exclusions of the members contract at time of service.

Insurance Liability for payment:
Your health insurance company will pay for services that it determines to be medically necessary, by their judgements. If your health insurance company determines that a particular service is not medically necessary, or that a particular service is not covered by your plan, your insurer will deny payment for that service.

Beneficiary Agreement:
I understand that my health insurance company may deny payment for massage therapy services. If my health insurance company denies payment, I agree to be personally and fully responsible for all payments. I also understand that if my health insurance company does make payment for services, I will be responsible for any co-payment, deductible, or coinsurance that applies.

No show/ Same day cancellation Fee:
We ask for at least 24 hour notice of cancellation so another client can receive care and get on the schedule. To honor both your time and the sustainability of the practice, If you cancel the same day with less than 24 hours notice, or do not show for your appointment, we have a $50 non-refundable fee. Please stay home if you are sick or have symptoms of being sick and call Peach studios or Terris Desk to cancel due to illness. Illnesses and emergencies are handled with compassion and discretion and we may waive the cancellation fee for you. Our No show fee waiver has a limit of two sessions though.
Illness and health Policy:
Please stay home if you have any active symptoms of a cold or flu: coughing, headache, fever. Please inform me of any skin conditions, lumps, bumps, and rashes. Unless you have a dermatologist's note for me, I may use gloves or avoid specific areas. Please inform me if you are pregnant. Please let me know if you have had or currently have blood clots, heart conditions, diabetes, cancer, brain injuries, surgeries, and/or blood pressure abnormalities. Some medical conditions I can not treat without a proper doctor's note. Some conditions are only local contraindications so I will avoid those areas. I use professional and educated discernment when determining if we need to adapt and change plans about your massage session. Please understand I have your best interests in mind to do no harm. It is incredibly rare I ever reschedule anyone but should I ever catch a bug I will extend the consideration to you to give you ample notice of rescheduling.

Late Arrival: If you arrive late, we will do our best to provide the most beneficial session possible within the remaining appointment time. Please allow time to arrive to pay for parking, and check in.
Parking and session duration:
The parking lot and street meters are patrolled by 3rd party companies. Park Mobile has options online and through their app to pay for parking. Both street meters and large QR code signs have options for different forms of payment. If you come into the office your therapist will assume you have paid for enough time to cover the duration of the appointment. Your therapist will verbally ask you if we can extend your appointment time without going over your paid parking sessions, and only do so if you agree. Tickets for unpaid parking are expensive and Peach Studios LLC is not responsible for any tickets you may receive while using the lot during your time in office and/or in an appointment.
Scope of Practice:
Alyssa at Peach Studios LLC has the proper licenses for providing massage with a very educated background in medical massage. She does not diagnose or offer suggestive ideas of diagnosis of any health concern. She provides therapeutic and relaxing massages with strong professional boundaries, Ethical boundaries, medical boundaries, and contract boundaries with the insurance groups she is contracted with. She understands power dynamics well, and can assure you that she cares to keep the power sense equal. There is no medical paternalism here.

Communication:
Peach Studios LLC operates without any traditional front desk staff. Please keep this in mind when calling the studio. Alyssa will return calls within 48 hours, but is able to respond to texts during business hours usually the same day. If you can’t reach Alyssa for immediate communication needs, like rescheduling of a session the same day, please reach out to Terri at Terri's desk.

Consent:
Massage is collaborative. Pressure, technique, and tools can be adapted and changed at any time for any reason at the discretion of the client. Clients can request modifications at any time. If a client doesn't communicate their preferences, Alyssa can read some body language, but otherwise she will not know if you need something different to happen. Alyssa does not take it personally, please speak up! unless you make an effort in letting her know, assume she doesn't know. Please indicate limitations and preferences of how you want your session to go, including pressure limits, range of motion, surgeries or injuries. She will also open the conversation sometimes to invite you to the conversation of what works for you.

Draping:
Washington State has laws on how massage therapists are required to drape sheets. Peach Studios LLC follows all of these laws.

Practitioner well being:
To provide attentive, high quality care, Peach Studios LLC reserves the right to reschedule appointments when practitioner illness, injury or unforeseen circumstances would compromise the quality or safety of treatment.

Mutual Respect Policy:
Peach Studios LLC is built on presence, trust, and compassion. We are committed to providing a safe and respectful environment for every person who enters the practice. The professional relationship in which both parties thrive- requires kindness, professionalism, and warm communication from both practitioners and clients equally. Harassment, discrimination, abusive or aggressive language, sexually implied commentary, or inappropriate behaviors will not be tolerated. Peach Studios LLC is a safe place for all.
Misconduct and No Dating:
Alyssa, the therapist at Peach Studios LLC, will not date any clients. She is kind, and friendly but has very strong professional boundaries. If you are confused why she is being nice, please know, it is within her job to be kind and she cares through a professional lens to help you feel safe. She seeks professional guidance expanding her education of ethics in client relations to have the healthiest understandings. She tracks client interaction behaviors based on a red, yellow, and green flag system. Red and Yellow flag behavior may result in termination of session appointments. Examples of red and yellow flag behavior: sexually explicit behavior; sexually suggestive behavior or conversations; removing draping and exposing private body areas; asking about her personal dating relationships; bringing cameras into treatment rooms. Red flag behavior is overtly inappropriate, yellow flag behavior is covertly disguised and not normal. Neither are tolerated. Immediate termination of visits are at the discretion of Alyssa.
